Acute Inpatient Case Manager (RN or MSW) – Michigan

 

Job Summary:
• Seeking an experienced Acute Inpatient Case Manager to support discharge planning and patient care coordination in a hospital setting. 
• Must be able to work independently with minimal training.
• Case ratio: 1:21–25 (based on patient risk factors)
• No telephonic or therapy experience considered

 

Requirements:
• 5+ years of acute inpatient case management experience
• Recent (within 2 years) hospital case management/discharge planning experience
• RN (MI license) or LMSW (MI license)
• BLS (AHA) certified
• Epic EMR and strong computer skills
• Bachelor’s degree required only if converting to permanent role
